Payment policy

Medical services are provided and billed by the office of Dr. David Feldman. Dr. Feldman participates in most of the major health insurance plans in the region. Following are the health plans which we currently accept.

If your plan is not listed, we will be unable to file a claim on your behalf. However, we can provide you with a form that you can submit to your insurance company to instruct them to send reimbursements directly to you.

Please be aware that with any health plans, there may be deductibles and copayments for which you could be responsible.

Questions about your bill

What billing or insurance information should I bring?

​You should check with your insurance company for any out of pocket deductibles or copays you have under your policy. You will be expected to pay any amounts which your insurance does not cover at the time of your visit.

How long will it take to get things settled with the insurance company?

Generally, it takes from 45 to 60 days to obtain payment from an insurance carrier.​

​Why am I getting separate bills from the lab, pathology, anesthesia and radiology physicians?

​Our office will varify your insurance and refer your care within your insurance network . Sometimes your plan contract with the insurer will not pay the full  allowed amount for a  medical service. Instead your contract with the insurer will allow the insurer to assign to you a copayment or deductible.Each lab or physician who provides professional services may send you a separate bill. These labs or physicians may include:

- -  A Lab that did a blood or urine test on you.

--Another physician who provided lab or pathology testing on a specimen you provided

--  An Anesthesiologist who provided services for your procedure

-- A radiologist who provided an interpetation of your images

​I received a notice that my insurance company has paid on my bill -- but I can't understand how they calculated their payment amount. Do you know?

​If we have received any such information from your insurance company, we'll be glad to share it with you. However, for answers to any questions about insurance payments, deductibles, or co-payments, you generally need to check with your insurance company.
